Certified Siding Installer Application: Step 3

Please read through the following points and agree to the handling and installation requirements provided in the Certified Installer Instructional Video and submit your application for approval.

• I agree that I have fully viewed the Certified Installer Instructional Video.
• I agree to transport, handle, store and install siding according to requirements in the Certified Installer Instructional Video.
• I understand that warranty will be void if transport, handling, storing and installing does not follow the requirements according to the Certified Installer Instructional Video.
• I understand that warranty will not be in place until my application is approved and I receive my Midwest Factory Finishes Certification.
• I understand that warranty will not be in place until homeowner receives all documentation included with siding.
• I understand warranty will be void if there is no documentation of siding in the original unit and original packaging showing peeling of paint.
